No healthcare is totally free. You pay your monthly premiums, co-pays, and a deductable. Most insurance (even Medicare plans) won't tell you the out-of-pocket expenses before a procedure. I've had patents tell me they've had treatment pre-authorized only to get a bill in the mail for thousands. It is so bad Congress passed a law to stop insurance companies and Hospitals from "surprise" bills. (it's called the 'no surprise act').
